INTRODUCTION To many Americans, childbirth is traditionally perceived as a physician attended event that occurs in a hospital setting. Although this is the dominant view of obstetric care in the United States, it is not the prevailing view of birth in most parts of the world. In nearly all other developed countries, midwives assume the central role in the management of normal pregnancy and birth. (1) While midwife-attended births are the norm elsewhere, the use of a midwife as a birth attendant or the choice of a home birth are often deemed archaic practices in the United States, (2) where most deliveries are attended by specialist obstetricians in hospitals.
